Service Level Agreement


A service level agreement is a document highlighting a contractual service wherein the service provider gives certain assurance regarding the standard of its service. It lays down the minimum criteria that the service body is expected to fulfil and deficiency in the expected quality is bound to set off litigation. Hence the document should be framed circumspectly.

Sample Service Level Agreement

This service level agreement infers the quality of the service to be met upon the execution of a legal contract between Divine Medical Hospital, hereby referred to as service contractor, and Boston Consultancies, hereby referred to as service provider.

The criteria of the contract which define the standard of the service are given below.  It must be noted here that if the service provider falls short of rendering the services up to the expected level, the service contractor will have the right to sue it.

  • The nurses and wardens who will be hired by the service contactor will be registered professionals of the American Medical Association and carry a valid service license and identity proof.
  • The equipments furnished by the service provider will be sterilized and the medicines supplied to the hospital wards will be up-to-date with recent manufacturing dates.
  • The contract will take effect from 6th June, 2011 and terminate on 6th March, 2012.
  • The staff members will not skip days and remain absent from work for 5 days at a stretch.

The parties agree to the aforesaid terms and hereby promise to fulfil these.
